2015 STD Treatment (Tx) Guide app

Description

The STD Treatment (Tx) Guidelines mobile app serves as a quick reference guide for doctors and related parties on the identification of and treatment for sexually transmitted diseases (STDs).

The app is a collaboration between CDC's Division of STD Prevention and the Informatics Innovation Unit (IIU) within CDC's Division of Public Health Information Dissemination (DPHID) - Center for Surveillance, Epidemiology and Laboratory Services (CSELS).

These guidelines for the treatment of persons who have or are at risk for sexually transmitted diseases (STDs) were updated by CDC after consultation with a group of professionals knowledgeable in the field of STDs who met in Atlanta on April 30–May 2, 2013. The information in this report updates the Sexually Transmitted Diseases Treatment Guidelines, 2010 (MMWR Recomm Rep 2010;59 [No. RR–12]). These updated guidelines discuss 1) alternative treatment regimens for Neisseria gonorrhoeae; 2) the use of nucleic acid amplification tests for the diagnosis of trichomoniasis; 3) alternative treatment options for genital warts; 4) the role of Mycoplasma genitalium in urethritis/cervicitis and treatment-related implications; 5) updated HPV vaccine recommendations and counseling messages; 6) the management of persons who are transgender; 7) annual testing for hepatitis C in persons with HIV infection; 8) updated recommendations for diagnostic evaluation of urethritis; and 9) retesting to detect repeat infection. Physicians and other health-care providers can use these guidelines to assist in the prevention and treatment of STDs.

Guideline on when to start antiretroviral therapy and on pre-exposure prophylaxis for HIV

Overview

This early-release guideline makes available two key recommendations that were developed during the revision process in 2015. First, antiretroviral therapy (ART) should be initiated in everyone living with HIV at any CD4 cell count. Second, the use of daily oral pre-exposure prophylaxis (PrEP) is recommended as a prevention choice for people at substantial risk of HIV infection as part of combination prevention approaches. The first of these recommendations is based on evidence from clinical trials and observational studies released since 2013 showing that earlier use of ART results in better clinical outcomes for people living with HIV compared with delayed treatment. The second recommendation is based on clinical trial results confirming the efficacy of the ARV drug tenofovir for use as PrEP to prevent people from acquiring HIV in a wide variety of settings and populations.

The recommendations in this guideline will form part of the revised consolidated guidelines on the use of ARV drugs for treating and preventing HIV infection to be published by WHO in 2016. The full update of the guidelines will consist of comprehensive clinical recommendations together with revised operational and service delivery guidance to support implementation.

Partner agencies support new guideline on antiretrovirals

Mark Dybul, Executive Director, The Global Fund

"We welcome the early release of this guideline. The two recommendations are critically important to moving us towards the fast-track treatment and prevention goals. Expanding access to treatment and prevention, especially for key populations and adolescent girls, is now a major global health challenge that requires our collective commitment and determination. We must embrace ambition if we are going to end HIV as a public health threat.”

Deborah L. Birx, U.S. Global AIDS Coordinator & U.S. Special Representative for Global Health Diplomacy

"PEPFAR applauds the World Health Organization on the release of their ‘Guideline on when to start antiretroviral therapy and on pre-exposure prophylaxis for HIV.’ This guideline expands life-saving treatment access to all persons living with HIV (PLHIV) and highlights new developments in HIV prevention, including PrEP.‎ These are transformative to epidemic control. Short of an HIV vaccine or cure, this gives us the critical tools we need to create an AIDS-free generation utilizing the FAST TRACK strategy. ‎ We have no excuses - it is up to us to seize this moment and chart a bold course together to end AIDS as a public health threat.‎"

Guidelines for the Use of Antiretroviral Agents in HIV-1-Infected Adults and Adolescents

Revisions to the May 1, 2014, version of the guidelines include key updates to several existing sections and the addition of two new tables. Significant updates are highlighted throughout the document.
Key Updates
The following are key updates to existing sections of the guidelines.
Since the last version of these guidelines, data from clinical trials and cohort studies, as well as experience in clinical practice, have prompted significant changes to the list of Recommended, Alternative, and Other regimens for treatment-naive patients (Table 6). Additionally, a new table, titled “Antiretroviral (ARV) Regimen Considerations as Initial Therapy Based on Specific Clinical Scenarios,” has been created to guide clinicians on the selection of an initial ARV regimen based on specific clinical scenarios and ARV-related considerations (Table 7).

Guidelines for the prevention, care and treatment of persons with chronic hepatitis B infection

Overview

These are the first World Health Organization (WHO) guidelines for the prevention, care and treatment of persons living with chronic hepatitis B (CHB) infection, and complement similar recently published guidance by WHO on the prevention, care and treatment of infection due to the hepatitis C virus (HCV).
The recommendations in these guidelines promote the use of simple, non-invasive diagnostic tests to assess the stage of liver disease and eligibility for treatment; prioritize treatment for those with most advanced liver disease and at greatest risk of mortality; and recommend the preferred use of nucleos(t)ide analogues with a high barrier to drug resistance (tenofovir and entecavir, and entecavir in children aged 2–11 years) for first- and second-line treatment. Recommendations for the treatment of HBV/HIV-coinfected persons are based on the WHO 2013 Consolidated guidelines on the use of antiretroviral drugs for treating and preventing HIV infection, which will be updated in 2015.
These recommendations provide opportunities to save lives, improve clinical outcomes of persons living with CHB, reduce HBV incidence and transmission, and stigma due to disease, but they also pose practical challenges to policymakers and implementers in low- and middle-income countries.
